24-HOUR CANCELLATION POLICY

A 24-hour cancellation policy is upheld. If the therapist is not notified about a cancellation 24 hours before the scheduled session, the full fee charge will be applied. 

PAYMENT

Cash, check, Zelle, and all major credit cards accepted for payment~ I DO accept HSA/FSA cards. Checks are made out to 'Mary Vice, LMFT’ whom I am working under supervision of.

INSURANCE

Currently, I do not take insurance (see below). I can provide you with an invoice to submit to your insurance provider for potential reimbursement. 

*** When therapy is managed through insurance, your personal information regarding mental health services are shared, as well as diagnoses, and often insurance companies control frequency of therapy, type of therapy, and whom can be treated. Because of this, I do not take insurance. If you would like to be reimbursed through insurance, some PPO’s may reimburse for our of network providers. You should contact your insurance company to ask them what your plan offers for out of network services. I am happy to provide you with an invoice to submit to your insurance provider for reimbursement.

MINORS

Minors in therapy are required in my practice to have a parent or legal guardian’s signature of consent. Should the parents of the minor(s) be divorced or separated with shared legal custody, consent from both parents is required.

*Please note, letters will not be written for custody or divorce cases.

CONFIDENTIALITY

For licensed mental health professionals, confidentiality is protected by state laws and the Health Insurance Portability and Accountability Act (HIPAA). This means that what clients discuss in therapy is kept between the client and therapist. Confidentiality is legally required to be waived in the cases in which the client poses an imminent risk to themselves or others, or when there is suspected child, elder, or dependent adult abuse. A court order by a judge can also require a breach in confidentiality. The client may choose to sign a release of information (ROI) should they like information to be exchanged between the therapist and outside providers, family members, practitioners/medical providers and schools.

CONFIDENTIALITY WITH MINORS

After the age of 12, minors are protected by confidentiality. In order to provide confidential information shared in therapy with parents or outside parties, the minor must consent (verbally) with parents and written with outside parties (such as schools).

HOW TO REACH ME

The best way to communicate with me is via email with questions, comments, or therapy-related content, but I also use texting and calling as well. I primarily reserve texting for appointment scheduling/rescheduling.
